The Health and Care Worker visa is a sub-category of the Skilled Worker route, designed for qualified professionals coming to work in the NHS, social care, or related fields. It offers faster processing, reduced fees, and — importantly — a route to settlement after 5 years.
Absence rules apply as standard
Health and Care Worker visa holders are subject to the same absence rules as other Skilled Worker visa holders:
- Maximum 180 days outside the UK in any rolling 12-month period
- Overall absence should be minimised
- Continuous residence of 5 years required for ILR
Shift work and annual leave
Many healthcare workers accrue significant annual leave, and it’s tempting to use it for extended trips home. Be careful: a four-week holiday plus a two-week trip later in the year gets you close to a third of your annual allowance.
Spread out your trips
Rather than two long absences per year, consider shorter, more frequent visits. This not only spreads your absence days but also helps with maintaining ties in the UK.
Training placements overseas
Some NHS roles involve short overseas placements or training abroad. These still count as days outside the UK. However, if the placement is formally required by your employer and directly related to your role, it may be treated favourably at the discretion stage — but don’t rely on this.
Carers and family emergencies
If a close family member abroad falls seriously ill, you may need to travel at short notice. Keep evidence (medical records, flight dates, communication with your employer) — these compelling circumstances can support a discretion request if you exceed the limits.
